Required Documentation
- Your child’s Birth Certificate (certified only; hospital certificate not accepted) or proof of birth.
- Proof of your address (FPL bill, lease agreement or property deed).
- Health Examination Form (DH 3040) including clinical TB screening/results.
Certificate of Immunization (DH 680) obtained through your doctor or local health provider. Kindergarteners require proof of two doses of measles and also must have completed the Hepatitis B series, as well as 2 varicella (chicken pox) vaccines or documentation of history of varicella disease. First graders require proof of two doses of measles and also must have completed the Hepatitis B series, as well as 2 varicella (chicken pox) vaccines or documentation of history of varicella disease.
